What is Support Coordination?
Support Coordination is an NDIS-funded service that helps people understand and use their NDIS plan to best support them in achieving their goals and live their life without limits.
A Support Coordinator works alongside Clients to help them understand their plan, identify their goals and connect with the services that will best support their needs.
How Support Coordinators help
Our Support Coordinators provide personalised support to help Clients get the most from their NDIS plan. This can include:
- Explaining how an NDIS plan works and how funding can be used
- Helping clients understand and manage their plan budget
- Connecting clients with appropriate service providers
- Supporting clients to prepare for NDIS plan reviews
- Assisting with requests for changes or additional supports
- Coordinating multiple services to ensure they work well together
Support Coordinators build ongoing relationships with clients, checking in regularly to ensure their supports are working well and adapting as needs change.
Specialist expertise in blindness and low vision
Our Support Coordinators have specialist knowledge and experience supporting people who are blind or have low vision.
This expertise helps them understand the unique circumstances clients may face and connect them with services that support independence, mobility and daily living.
While our team specialises in blindness and low vision, they can also support clients with a broad range of needs, including complex care arrangements, coordinating multiple services and assisting participants in finding, modifying, or living independently in suitable accommodation.
