Service Connection Program for vulnerable older people in the East Melbourne area

As part of the care finder network, Access Health and Community would like to introduce its Service Connection Program. This is a free service providing short and long-term support for vulnerable older people and those who are homeless or at risk of homelessness. Their Connectors help people understand what aged care services are available, assist in setting up aged care assessments and navigating the My Aged Care portal, connect Clients with services and engage them in the community.

Eligibility criteria includes the vulnerable older person not having a support person or not trusting their support person, having communication barriers, including low literacy, being reluctant to engage with aged care or government services, having difficulty understanding information and make informed decisions or are in an unsafe situation without services in place to support them.

The Service Connection Program is available to people living in Boroondara, Banyule, Nillumbik, Whitehorse and the lower areas of Murrundindi.
